In-Home Comprehensive Care and Safety Assessment
$150 (one-time fee)

Meet with potential clients and/or their families to discuss goals and objectives.

Assess the physical, emotional, mental, and social conditions of the client to determine the feasibility of achieving their goals.
* Develop a holistic, personalized, and moveable plan outlining the responsibilities of both Peninsula Homecare Cooperative and the family in achieving the ongoing care of the client. .
* Recommend and facilitate modifications to the home to enhance safety and accident prevention

Homecare Service Rates:
Level 1 - $40 per hour for shifts lasting 3 or more hours
Level 2 - $43 per hour for shifts lasting between 2 and 3 hours
Level 3 - $52 for shifts lasting between 1 and 2 hours
(minimum 1 hour)

Specialized Care - (1) Households Where 2 or More Residents Require Care, (2) Short-Term Urgent or Eleventh-Hour Emergency Care:
Level 1 - $43 per hour for shifts lasting 3 or more hours
Level 2 - $46 per hour for shifts lasting between 2 and 3 hours
Level 3 - $55 for shifts lasting between 1 and 2 hours (minimum 1 hour)

Long-Term Care Insurance - $75 per month
We will work with your LTCI provider to reimburse your home care costs. This includes submitting necessary documents on a bi-weekly basis.
